Cover Story: Myelofibrosis is a progressive myeloproliferative neoplasm characterized by splenomegaly, cytopenias, and debilitating symptoms. While ruxolitinib remains a first-line treatment, many patients discontinue due to intolerance or resistance. This review outlines current and emerging therapeutic options following ruxolitinib failure, including fedratinib, momelotinib, pacritinib, and novel agents in clinical trials. A personalized, risk-adapted approach is essential for optimizing outcomes in this complex disease. View this paper
