Cover Story: Liver fibrosis, driven primarily by activated hepatic stellate cells (HSCs) and other fibrogenic cells, remains a major global health burden. Pirfenidone (PFD) shows clinical promise against liver fibrosis, yet its mechanism remains elusive. We show that PFD reprograms HSC amino acid metabolism by suppressing ATF4 nuclear accumulation, downregulating GLS1, SHMT2, and P5CS—the enzymes governing glutaminolysis, glycine synthesis, and proline production. This dual action curtails precursors for collagen biosynthesis while depleting glutathione, the master cellular antioxidant. Consequently, activated HSCs lose their ferroptosis resistance and undergo synergistic death with the ferroptosis inducer erastin, an effect abrogated by ferrostatin-1 and N-acetylcysteine. These findings rationalize PFD-based combination antifibrotic therapies. View this paper
