Cover Story: Subaxial cervical spine fracture-dislocations are severe injuries associated with spinal instability and potential neurological impairment. While an anterior approach is often recommended in the presence of traumatic disc lesions, the optimal surgical strategy remains controversial. In this retrospective single-center cohort study, 92 patients treated between 2005 and 2025 were analyzed with regard to neurological and radiological outcomes after anterior or posterior stabilization. Neurological recovery was comparable between both approaches, with deterioration occurring in only one patient per group. However, secondary posterior stabilization was required in nine patients initially treated anteriorly. These findings support a personalized, pathology-oriented treatment strategy rather than selecting the surgical approach solely based on the presence of traumatic disc protrusion. View this paper
