Special Issue Information

Dear Colleagues,

Inflammatory bowel disease (IBD), including ulcerative colitis and Crohn’s disease, is a chronic and heterogeneous condition shaped by complex interactions among genetic susceptibility, environmental factors, host immunity, and gut microbiome. Recent advances highlight dysregulated immune responses, altered microRNA expression, and characteristic microbial dysbiosis in IBD, marked by increases in pro-inflammatory and mucin-degrading bacteria and reductions in short-chain fatty acid-producing species that support intestinal health. Diet has emerged as a key environmental modifier of disease activity, and modern “omics” approaches now provide deeper insight into how genetics, nutrition, and the microbiome collectively influence disease mechanisms.

Up to 25% of IBD cases occur in childhood, often with more extensive and severe presentations. Although effective therapies exist, substantial variation in disease phenotype, progression, and treatment response underscores the need for personalized approaches. Predicting disease courses at diagnosis and tailoring therapy based on individualized risk and responsiveness have the potential to improve outcomes while minimizing toxicity. Emerging tools, including multi-omic profiling, machine learning, and artificial intelligence, offer new opportunities for patient stratification and precision dietary and medical interventions.

This Special Issue aims to highlight recent advances in personalized care for inflammatory disorders, with a focus on dietary strategies, data-driven patient stratification, and the integration of multi-omic and AI-based approaches to enhance IBD management.
