Cover Story: Delivering therapeutics to the brain remains a major challenge because the blood–brain barrier prevents most drugs from reaching tumors and other central nervous system diseases. Engineered exosomes have emerged as promising natural nanocarriers capable of crossing this barrier while transporting proteins, nucleic acids, drugs, and imaging agents. This review examines the mechanisms of exosome biogenesis, cargo loading, surface engineering, and targeted delivery, with particular emphasis on glioblastoma. We also discuss theragnostic applications, translational challenges, and emerging strategies to improve manufacturing, targeting, and clinical implementation. Together, these advances highlight the growing potential of engineered exosomes as a platform for precision neuro-oncology and CNS therapeutics. View this paper
