Cover Story: The diagnosis and treatment of myocarditis is still a major clinical challenge. Despite the development of new imaging techniques, endomyocardial biopsy remains the gold standard for the accurate diagnosis of inflammatory heart disease. With the introduction and continued development of inflammatory diagnostics in combination with viral nucleic acid testing, myocarditis diagnostics have improved significantly since its introduction. Together with new technologies such as miRNA and gene expression profiling, quantification of specific immune cell markers, and determination of viral activity, diagnostic accuracy and patient prognosis will continue to improve in the future. Here, we highlight the current state of knowledge on the pathogenesis and diagnosis of inflammatory myocardial diseases, and present future perspectives for more in-depth biopsy diagnostics and personalized therapy.
