Cover Story: PLGA nanocarriers can be precisely engineered, but their biological identity changes once they encounter blood and tissue fluids. Proteins adsorb, exchange, and reorganize on the particle surface, forming a dynamic corona that can mask targeting ligands, redirect cellular uptake, accelerate immune recognition, or even enable beneficial delivery pathways. This review connects nanocarrier design with corona formation, cellular and tissue interactions, and in vivo fate. By showing why promising laboratory properties may not predict biological performance, it argues that PLGA systems should be evaluated for how reliably their intended functions survive realistic biological exposure.
