Cover Story: Cardiac surgery remains a paradigm of perioperative complexity, where ischemia–reperfusion injury and systemic inflammation critically influence outcomes. Beyond their hypnotic effects, volatile anesthetics have emerged as modulators of cellular stress responses, activating mitochondrial pathways, reducing oxidative injury, and attenuating inflammatory cascades. While experimental evidence supports their organ-protective properties, clinical translation remains heterogeneous. This review explores the mechanistic foundations and clinical implications of volatile anesthesia in contemporary cardiac surgery, emphasizing the need for patient-specific strategies and the transition toward precision perioperative care. View this paper
