Cover Story: MDR/XDR Gram-negative bacteria increasingly compromise the treatment of severe ocular infections. We examined whether the membrane-active agents colistin and Mel4 could enhance conventional antibiotics against clinical ocular isolates of Pseudomonas aeruginosa, Acinetobacter baumannii and Klebsiella pneumoniae. Activity was assessed using MIC/MBC, checkerboard, time-kill, biofilm-disruption, membrane-permeability, and mammalian-toxicity assays. Of the 102 combinations tested, 45 were synergistic and reduced antibiotic MICs up to 4096-fold. Combinations produced rapid bactericidal activity (>3 log10 reduction within 3 h), while ciprofloxacin plus Mel4 disrupted preformed biofilms by 62–92% with low toxicity. Membrane-active adjuvants may therefore provide a useful strategy for severe MDR/XDR ocular infections. View this paper
