Cover Story: Elevated lipoprotein(a) [Lp(a)] is a major independent cardiovascular risk factor, yet its impact on HDL quality remains unclear. In this cross-sectional study, we investigate the association between high Lp(a) levels and the distribution and functional quality of HDL subfractions. Our findings reveal a morphological shift in HDL subpopulations toward smaller, dysfunctional particles. This structural change coexists with a significant loss of HDL antioxidant function. As Lp(a) acts as a carrier of oxidized phospholipids (oxPL), elevated levels may overload and compromise HDL defense mechanisms. By linking high Lp(a) levels to impaired HDL functionality, our findings underscore the importance of assessing qualitative HDL markers to better evaluate residual cardiovascular risk. View this paper
