Cover Story: A series of global mechanisms contribute to systemic iron deficiency in heart failure and its consequences. The causes can be divided into the reduced intake, decreased absorption, and increased loss of iron. Reduced intake could be due to decreased absolute amounts of iron or low iron bioavailability. Reduced absorption is thought to be due to intestinal oedema, increased intestinal pH, increased hepcidin levels, and intestinal hypoperfusion, among others. Increased losses are mainly due to gastro-intestinal bleeding. Iron deficiency results in reduced organ function of the heart and skeletal muscles, anaemia, impaired immune responses, deranged metabolism, and kidney injury. Iron deficiency is associated with impaired exercise tolerance, increased heart failure hospitalisations, and mortality. View this paper
