Cover Story: The advent of high-sensitivity assays of cardiac troponin, which plays a central role in evaluating patients with angina, has come at the cost of decreased specificity. More than ever, interpreting troponin elevation requires clinical judgment and careful integration of findings from electrocardiography, echocardiography, the physical exam and patient interview, and other imaging and laboratory data. This state-of-the-art review presents a comprehensive survey of the prevalence, mechanisms, and prognostic implications of troponin elevations in each cardiac and non-cardiac etiology to assist the clinician in distinguishing between presentations of myocardial ischemia, myocardial infarction, and myocardial injury.
