Cover Story: Coronary artery disease (CAD) is the leading cause of death worldwide. The immune system plays a complex role in CAD, initiating an inflammatory response to damage or injury to arterial walls. This response can become chronic and lead to plaque formation and atherosclerosis. Neutrophiles, macrophages, B and T lymphocytes, and NKT cells are key in the immune response. Studies have revealed the new roles of endothelial cells and vascular smooth muscle cells. Research is ongoing to explore immunomodulatory therapies to reduce inflammation and atherosclerosis. This review explores the pathogenic relationship between immunity and atherosclerosis, potential therapeutic strategies, and immunomodulatory therapies targeting the immune system to reduce inflammation.
