Cover Story: This comprehensive review explores left ventricular thrombus (LVT) in ischemic and non-ischemic cardiomyopathies including Takotsubo syndrome, hypertrophic cardiomyopathy, eosinophilic myocarditis, Chagas disease, cardiac amyloidosis, peripartum cardiomyopathy and LVT associated with oncological treatment. Diagnostic tools like transthoracic echocardiography and cardiac magnetic resonance imaging are crucial, despite varying sensitivity. Anticoagulation strategies, such as direct oral anticoagulants, are highlighted for their potential, though more research is needed on their efficacy and safety, especially in non-ischemic cardiopathies.
