Cover Story: Lymph node metastasis (LNM) may occur despite no lymphatic invasion being detectable by conventional assessment. We evaluated periarterial/perivenous invasion (periA/V), defined as tumor nests in direct contact with perivascular elastic fibers on Elastica–van Gieson (EVG)-stained sections, as a surrogate marker associated with LNM in 213 cases of invasive breast carcinoma of no special type. In pT1 tumors, periA/V showed high sensitivity and high negative predictive value for LNM, with higher sensitivity than conventional lymphatic invasion markers. In multivariate analyses, periA/V remained independently associated with LNM. In this cohort, periA/V negativity in pT1 tumors was observed only in node-negative cases. These preliminary findings suggest that EVG-based periA/V may contribute to LNM risk assessment in early-stage breast cancer. View this paper
