Cover Story: Immune thrombocytopenia (ITP) results from a peripheral platelet destruction and an inappropriate bone marrow production. Recent advances have highlighted the multiple pathways involved in ITP pathogenesis such as antibody-dependent cell phagocytosis, complement dependent cytotoxicity, CD8 T cell mediated cytotoxicity, platelet desialylation and insufficient production of thrombopoietin. New upcoming therapies which specifically target one or multiple mechanisms involved in ITP, such as Syk inhibitors, IgG Fc receptor (FcγR) inhibitors, complement inhibitors and neonatal Fc receptor (FcRn) blockers are about to deeply modify the management of ITP, currently based on corticosteroids, intravenous immunoglobulins, thrombopoietin receptor agonists, immunosuppressants and splenectomy.
