Cover Story: Angiotensin-converting enzyme (ACE) inhibitors are extensively utilized antihypertensive drugs that are generally well-tolerated. They inhibit the production of the pressor hormone angiotensin II from the inactive precursor angiotensin I but also protect the vasodilator peptide bradykinin from degradation by the peptidase ACE. A rare but severe adverse effect of ACE inhibitors is oro-facial angioedema, which may be mediated by bradykinin-related peptides. In the current issue, Marceau reviews the pathophysiology of ACE inhibitor-induced angioedema, suggesting that excessive production of kinins is initiated by tissue kallikrein from salivary glands, leading to localized edema. In contrast, plasma kallikrein is central in the pathophysiology of hereditary angioedema. These insights bear significance for the therapy of both conditions. View this paper
