Cover Story: Fully digital guided implant surgery is accurate but costly, software-dependent, and tied to specialized laboratory workflows. This study introduces a prosthesis-based, 3D printed guide for flapless implant placement in the edentulous maxilla. Its modular design embeds repositionable polyether–ether–ketone (PEEK) rails and interchangeable angulation sleeves in a duplicate denture, letting clinicians adjust sleeve position and implant trajectory intraoperatively without re-printing the guide. In a prospective split-mouth pilot, the workflow proved clinically feasible, gave early outcomes consistent with published series, and produced marked gains in oral-health-related quality of life. By preserving prosthetically driven placement while reducing the infrastructure required, it offers a cost-conscious alternative to proprietary platforms. View this paper
