Cellular and Molecular Mechanisms in Sepsis: Pathogenesis, Immune Response, and Therapeutic Strategies

Sepsis is a complex and dynamic biological phenomenon involving over 200 cytokines, numerous receptors—many exhibiting genetic polymorphisms—and a wide array of mediator–cell interactions. Despite decades of progress in molecular biology and the advent of high-resolution analytical tools, the intricate regulatory landscape of sepsis remains incompletely understood. The same microbial trigger can elicit divergent molecular and cellular responses depending on the context, reflecting the influence of host genetics, immune history, metabolic state, and other intrinsic variables. Additional layers of complexity arise from interactions among signaling pathways, feedback loops, and epigenetic modulators, making it challenging to fully characterize the systemic immune response.

This Cells Special Issue invites original research articles and comprehensive reviews that explore the molecular and cellular foundations of sepsis. Topics of interest include, but are not limited to, the following: cytokine signaling pathways, receptor dynamics and polymorphisms, transcriptional and epigenetic regulation, non-coding RNA networks, metabolic reprogramming, redox signaling, and systems biology approaches to modeling immune network behavior. Studies utilizing cutting-edge techniques—such as single-cell transcriptomics, spatial omics, proteomics, and functional genomics—are especially encouraged. By uncovering the cellular circuits and molecular regulators that govern immune activation and dysregulation in sepsis, this Special Issue aims to deepen our understanding of inflammatory complexity and reveal new principles of immune system dynamics under extreme stress.
