Cover Story: Ischemic stroke is the first cause of disability and second cause of death around the world. Thrombolysis with intravenous rt-PA is the most commonly administered fibrinolytic medical therapy. The use of different HPLC methodologies evaluating plasma samples from 108 patients treated and non-treated with rt-PA has allowed us to find that a group of highly hydrophilic compounds, with a molecular mass of 821 Da (determined by ESI-MS), were present in plasma samples of 80% of the treated patients soon after receiving treatment as the result of the rt-PA thrombolytic effect. A significant association was found between the presence of these compounds and the good neurological evolution of patients as 70% of patients yielding these compounds had early neurological improvement.
