Cover Story: Cardiac resynchronisation therapy is an important part of treatment for patients with heart failure. Traditionally, resynchronisation is achieved by biventricular pacing including a right ventricular pacing electrode in combination with left ventricular pacing via a coronary sinus contributary. It is of utmost importance to reach the area of latest mechanical activation. Achieving the optimal lead position via the coronary sinus can be difficult due to coronary sinus anatomy, scar tissue, and the proximity of the phrenic nerve. Minimally invasive epicardial lead placement via video-assisted thoracoscopic surgery is often used for failed coronary sinus lead placement. We show the long-term comparison of these electrodes with traditionally placed coronary sinus electrodes and demonstrate excellent performance of epicardial electrodes used for cardiac resynchronisation therapy.
