Cover Story: Stereotactic body radiation therapy (SBRT) is a safe treatment option for localized prostate cancer, providing precise radiation doses to the tumor. In extreme-hypofractionated regimens, where intrafraction prostate displacement can impact treatment efficacy, real-time monitoring is employed to avoid the excessive exposure of healthy tissue or insufficient target coverage, but with a continuous investment of resources and time. Our approach focuses on reducing intrafraction motion impact by shortening delivery time while maintaining treatment quality. This involves optimal patient preparation, flattening-filter-free beams, appropriate margins, and one therapy arc. This study aims to comprehensively present data on optimizing linac-based single-arc SBRT for localized prostate cancer. View this paper
