Dear Colleagues,
Despite substantial advances in medical therapy and device support, heart failure remains a leading cause of morbidity, mortality, and healthcare burden worldwide. Current clinical and surgical strategies have improved outcomes, yet significant gaps persist in diagnosis, patient stratification, and tailored management across the heterogeneous spectrum of heart failure phenotypes. Moreover, innovative surgical interventions, mechanical circulatory support, and integration with cutting-edge pharmacologic therapies continue to challenge traditional care pathways. This Special Issue aims to bring together original research, comprehensive reviews, and clinical perspectives that address the core clinical and surgical challenges in heart failure management. We particularly welcome submissions that advance understanding of pathophysiology, evaluate emerging therapeutic strategies (including novel surgical, interventional, or hybrid approaches), optimize perioperative and long-term patient care, and propose solutions to unmet clinical needs. By highlighting both clinical innovations and surgical perspectives, this Special Issue collection seeks to foster multidisciplinary collaboration and guide future research directions toward improved outcomes and personalized care for patients with heart failure. We encourage authors across cardiology, surgery, critical care, and allied disciplines to contribute to this evolving field.

Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-anonymized peer-review process.
