Cover Story: Gender-affirming hormone therapy (GAHT) is an essential component of care for many transgender individuals, but its long-term use involves therapeutic considerations beyond hormone prescribing. This systematic review synthesizes evidence on effects, adverse events, monitoring requirements, and medication-related issues associated with GAHT, with particular attention to community pharmacy practice. Across 116 studies, findings highlight cardiovascular and cardiometabolic risks, dermatological and gynecological effects, musculoskeletal and metabolic changes, pharmacological considerations, and psychiatric outcomes. By translating this evidence into clinically relevant issues, the review highlights opportunities for community pharmacists to contribute to medication safety, monitoring, counselling, and the coordination of care. View this paper
