Cover Story: The two main types of studies used to assess seasonal influenza vaccine (SIV) performance are randomized controlled trials (RCTs) and observational studies, mainly, test negative designs (TNDs). RCTs are the gold standard for licensing of use, and TNDs are the main tool for monitoring the annual effectiveness of the SIV. In this paper, it was possible to compare the assessment of the trivalent inactivated vaccines by both designs. This is especially important as several papers adverts for the variety sources of bias that may affect TNDs. Although there are several systematic reviews addressing the performance of the SIV, none have yet addressed the reasons for different effectiveness estimates of the same SIV. It was found that a match between the vaccine and the most prevalent circulating strains and the laboratorial method for confirmation of influenza are important sources of bias. View this paper
