Cover Story: Modern bacterial diagnostics move at two speeds. PCR can detect a pathogen’s genetic signature within hours, enabling earlier clinical action when time matters; culture and antimicrobial susceptibility testing reveal whether organisms are viable and how they respond to treatment. This review reframes these methods, not as rivals, but as complementary views of microbial reality. It shows why a cycle-threshold value is an assay-specific signal—not a universal measure of live bacterial burden—and why PCR–culture discordance can be biologically informative. By uniting rapid molecular detection with phenotypic confirmation and stewardship-aware interpretation, the work offers a practical framework for decisions that are faster, biologically grounded, and more precise. View this paper
