With the identification of a great number of neurotransmitters and neuroreceptors in the last few years, breakthroughs in neuroanatomy and neurosciences have progressively complicated the hypothesis of dopaminergic D2 hyperactivity in schizophrenia in favour of malfunctioning of the dopaminergic tracts. The clear demonstration of the efficacity of clozapine as compared with the other neuroleptics for resistant schizophrenia, as well as the fact that it improves negative symptoms without provoking extrapyramidal effects, have been the starting point for research to find new antipsychotic molecules. Three antipsychotic substances are currently in the process of being registered. These are sertindole, seroquel and olanzapine.The principal pharmacological characteristics and the initial clinical research on these three substances show that they have antipsychotic properties which act on both positive and negative symptoms of schizophrenia and that tend to provoke fewer extrapyramidal effects than classic neuroleptics do Full article

The psychiatric disease of Vincent van Gogh has been object of several psychopathological analyses. In an article which has met little advertence until now, Karl Leonhard showed that the symptoms of the disorder well match the diagnosis of an anxietyelation psychosis. In a comprehensive research of the historical documents it was confirmed that, based on all signs and symptoms as well as on the course, the case of van Gogh is typical for this subgroup of the cycloid psychoses. Leonhard showed that many aspects of van Gogh’s life chart indicate a personality with easily excitable affects. In the present paper, those passages of the letters Vincent’s to his brother Theo and those statements of contemporaries which allow conclusions about his psychological state were compilated, and a temptative interpretation in the form of a character profile is proposed. Based on the documents, it is shown that the personality of van Gogh is compatible with the effusive character described by Leonhard, and is often found in the premorbid personalities of cycloid psychoses. We discuss various aspects on an individual level and describe major features contributing to the emergence of a psychological trauma. Family dynamics, relationships and intergenerational characteristics of families where traumata take place are explored.The emotional responses of children to trauma and their long-term effects are reviewed as well as the posttraumatic consequences and developmental disorders. This paper points out possible relationships between the attachment theory and different concepts of family therapy. In a second step, other therapeutic approaches are discussed from this point of view in order to derive a frame of reference for the interpretation of spontaneous and therapeutic developmental processes also for adults. Implications are of high actuality especially for Social Psychiatry: A conceptualisation of therapy and research in this field based on the socio-emotional experiences in interpersonal relationships seems the most adequate to understand developmental processes but also the ambivalence and avoidance of those patients. Up to now there has not been made much use by brief therapy for patients over 60 years. A psychodynamic treatment concept has to orientate itself on continuing unconscious innerpsychic as well as on intra- and intergenerative conflicts,on emerging developmental tasks and on special conditions of aging (cumulating threats and losses, narcistic insults, increasing importance of the body and approaching of the end of life). Good therapeutic possibilities are demonstrated by three treatment examples. Full article
