Healthcare and Healthcare Economics in an Aging World: Adapting to Demographic Change

The global demographic shift toward aging populations is one of the most significant challenges of the 21st century, with profound implications not only for healthcare delivery but also for the economic sustainability of health systems. By 2050, it is projected that individuals aged 65 and older will constitute nearly one-fifth of the global population. In many countries, this demographic shift is accompanied by declining birth rates, leading to a shrinking workforce and a growing proportion of retirees. This imbalance poses critical questions about how to fund and deliver healthcare to a population that increasingly depends on, but may no longer directly contribute to, the healthcare system.

The aging process brings unique healthcare challenges, such as a rise in chronic illnesses, multi-morbidity, and increased demand for long-term care services. These trends require not only a transformation of clinical care models but also a rethinking of the economic frameworks that underpin healthcare systems. Traditionally, health systems have relied on contributions from a younger working population to fund care for older generations. However, as dependency ratios rise, this model is under growing strain, particularly in countries with publicly funded healthcare or social insurance schemes.

This Special Issue, "Healthcare and Healthcare Economics in an Aging World: Adapting to Demographic Change", seeks to address the dual challenges of delivering high-quality care to aging populations while ensuring the economic sustainability of healthcare systems. It will explore how healthcare providers, policymakers, and researchers can innovate to meet the demands of this demographic transition.

Key questions to be addressed include the following:

• How can healthcare systems adapt to meet the needs of an aging population while remaining financially sustainable?
• What policy measures can be implemented to balance intergenerational equity in healthcare funding?
• How can technology and innovation reduce healthcare costs while improving outcomes for older adults?
• What role do private sector partnerships, taxation reforms, and alternative funding mechanisms play in alleviating the economic burden of aging populations?
• How can economic strategies address the increasing caregiving burden placed on unpaid family members and informal caregivers?

This issue will also highlight the ethical dimensions of healthcare resource allocation in aging societies, addressing concerns such as rationing care, prioritizing treatments, and the potential economic marginalization of older adults.

By bringing together research from the fields of health economics, public policy, gerontology, and healthcare innovation, this Special Issue aims to provide a comprehensive understanding of the intersection between aging populations and economic sustainability. It will offer actionable insights for stakeholders navigating the complexities of demographic change, including policymakers, healthcare administrators, and researchers.

Healthcare and economics in an aging world are both a critical reflection and a forward-looking guide for ensuring that healthcare systems can deliver compassionate, equitable, and financially viable care for aging populations.
