Cover Story: Acute respiratory distress syndrome (ARDS) is characterized by hypoxemic respiratory failure and bilateral pulmonary infiltrates not fully explained by cardiac failure or hypervolemia but linked to alveolo-capillary barrier dysfunction, dysregulated inflammation, immune abnormalities, and responses to mechanical stimuli. Nearly 60 years after its first description, effective treatments remain largely supportive, and mortality is high—possibly due to distinct ARDS subphenotypes. This heterogeneity spans physiological and clinical features, etiology, timing, inflammatory status, biomarkers, and genetic factors. Improving the identification and characterization of discrete subphenotypes using clinical, biological, physiological, and radiographic criteria may enhance understanding of ARDS pathophysiology and enable more personalized therapeutic strategies. View this paper
