Cover Story: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a chronic, debilitating disease with unknown origin characterized by severe fatigue, post-exertional malaise, chronic pain, and cognitive dysfunctions. Gastrointestinal disorders, such as dysbiosis and leaky gut, are strictly linked to the central nervous system homeostasis and function, through the microbiota–gut–brain axis. A link between an unbalanced intestinal bacteria composition, increased gut permeability, chronic basal inflammation, and ME/CFS cognitive symptoms can be established. In our article, we review the role of gastrointestinal dysfunction in disease pathogenesis and progression, and we discuss the therapeutic potential of treating the gut to alleviate ME/CFS symptoms holistically.
