Cover Story: Venetoclax is a highly selective and effective B-cell lymphoma-2 (BCL2) inhibitor, with a unique ability to reinstate the apoptotic potential of cancer cells. With its full potential yet to be explored and a promising risk–benefit profile, it has proven to be paradigm shifting in a range of haematological malignancies. Despite this, venetoclax is not curative, and treatment is inevitably hamstrung by disease relapse over time. Ongoing research continues to shed light on the complex genomic and epigenomic environment that gives rise to disease resistance. Understanding these mechanisms further will aid rational drug combinations to improve long-term outcomes in patients on venetoclax therapy. View this paper
