Cover Story: Acute traumatic aortic injury is a rare but life-threatening consequence of blunt trauma in which rapid imaging assessment is critical. This review presents a practical radiologist-focused approach centered on CT angiography as the first-line modality for diagnosis, grading, treatment planning, and follow-up. It highlights the direct and indirect signs of injury, key mimics and artifacts that may lead to false-positive interpretation, and imaging features that guide endovascular or conservative management. By integrating protocol optimization, diagnostic pitfalls, and post-treatment surveillance, the review emphasizes the central role of imaging in timely multidisciplinary care. View this paper
