Cover Story: The recent publication of the landmark randomized multicenter OPTIMUM trial, concluding that micro-ultrasound is non-inferior to MRI/conventional ultrasonography fusion biopsy for the detection of clinically significant prostate cancer, marks a major milestone in the adoption of this novel imaging modality. Micro-ultrasound offers a promising alternative to MRI, with theoretical advantages that include lower upfront and ongoing maintenance costs and real-time image interpretation by the physician operator. In the wake of OPTIMUM, this review examines the key studies leading up to this seminal trial and evaluates whether the current evidence base for micro-ultrasound is comparable to that which established MRI as the gold-standard imaging modality in the AUA’s current prostate cancer detection guidelines. View this paper
