Cover Story: This review examines occupational eye lens radiation exposure during transcatheter aortic valve implantation (TAVI), a fluoroscopy-guided procedure increasingly performed in hybrid operating rooms. As TAVI expands worldwide, attention to staff radiation safety becomes ever more important, particularly for the eye lens—one of the most radiosensitive tissues. The article summarizes current evidence on operator and staff eye dose, the influence of procedural access, operator position, shielding, and imaging parameters, and the main determinants of scatter radiation. It also outlines practical dose-reduction strategies, including optimized fluoroscopy settings, reduced cine and DSA use, careful C-arm angulation, protective eyewear, ceiling-mounted shields, and dedicated eye-level dosimetry for TAVI teams. View this paper
