Cover Story: Musculoskeletal involvement is one of the most common manifestations of systemic lupus erythematosus (SLE), affecting up to 90% of patients and representing the onset symptom in 60–80% of cases. SLE arthritis can be classified into three different subtypes, with different prevalences and biomarkers. The most frequent clinical phenotype is represented by nondeforming nonerosive polyarthritis; more rarely, radiologically nonerosive but deforming arthropathy (Jaccoud’s arthropathy) and rheumatoid-like erosive arthritis (rhupus) can be observed. Identifying the pathogenetic mechanisms underlying the development of musculoskeletal manifestations is crucial for developing therapeutic strategies with which to suppress synovial inflammation, prevent erosions as well as deformities, and improve SLE patients’ quality of life.
