Cover Story: Metabolic acidosis is a common disorder in CKD patients. Under physiological conditions, arterial bicarbonate level is higher in arterial than in venous blood due to the production of endogenous acids in tissues. In the lungs, bicarbonate is regenerated. We found that arteriovenous gradient of bicarbonate (AVΔ-HCO3−) positively correlated with eGFR. In patients with advanced CKD, there is practically no arteriovenous difference in bicarbonate concentration. The value of AVΔ-HCO3− may facilitate decision of bicarbonate supplementation or renal replacement therapy start in doubtful situations. View this paper.
