Cover Story: Migraine with aura involves cortical spreading depression, a wave of neuronal depolarization triggered by ion channel dysfunction, calcium surges, and excess glutamate. Despite its prevalence, MwA’s pathophysiology is incompletely understood. fMRI reveals visual cortex hyperactivation during auras and hypoactivity interictally, suggesting chronic excitability shifts. Genetic variants implicate calcium signaling, while advanced neuroimaging shows persistent network dysfunction. Distinct visual cortex activation patterns differentiate MwA from other headaches, suggesting diagnostic biomarker potential. View this paper
