Clinical Advances in Chronic Lymphocytic Leukemia: Focus on Elderly Frail Patients

The management of chronic lymphocytic leukemia has seen outstanding advances over the past 10 years. New available molecules are accessible to all patients, regardless of age frailty, with very satisfactory results (i.e., bcl2 inhibitors, BTK inhibitors, non-covalent BTK inhibitors, BTK degraders, and antiCD20). Guidelines have evolved from stratification based on age and degree of frailty to stratification based on molecular characteristics such as TP53 and IgVH mutation status. Currently, there are no specific recommendations for the management of patients over 80 years of age or those who are very frail. This population is underrepresented in clinical trials evaluating new therapies. The objective of this review is to assess the tolerability and efficacy of currently available treatments and attempt to develop recommendations for this specific population considering quality of life and patient’s wishes.
