Dear Colleagues,
Serum Amyloid A (SAA) is an acute-phase protein that increases more than 1000-fold in circulation during acute inflammation. Despite being identified over 50 years ago, its physiological functions remain unclear. While undetectable in healthy individuals, SAA levels rise modestly and persist in chronic inflammatory diseases until the condition resolves.
Preclinical studies suggest that SAA exacerbates chronic inflammatory diseases such as atherosclerosis and abdominal aortic aneurysm. In contrast, SAA is thought to play a protective role during acute inflammation, such as sepsis. This distinction suggests that the transient elevation of SAA in acute inflammation is a physiological defense mechanism, whereas its persistent elevation in chronic conditions represents dysregulation and may be harmful.
However, significant controversies persist in SAA research. Many in vitro findings have not been reproducible in vivo, raising concerns about their physiological relevance. Additionally, as SAA circulates primarily bound to high-density lipoprotein (HDL), which masks many of its properties, it remains unclear how SAA exerts its effects in vivo. Given these uncertainties, it is crucial to consolidate existing evidence and establish validated insights into SAA’s functions.
