Cover Story: The styloid process is usually remembered for Eagle syndrome, yet it lies in one of the most crowded corridors of the neck. Between the styloid process and the C1 transverse process run the internal jugular vein, the carotid arteries, and the lower cranial nerves. This review shows that symptoms depend less on styloid length than on three-dimensional orientation, proximity to vessels and nerves, and head and jaw movement. We propose a phenotype-based framework (neural, arterial, venous and mixed) that connects each presentation to the compressed structure. Three-dimensional CT angiography/venography maps the anatomy, while position-dependent imaging and catheter manometry clarify venous cases. A cautious “styloidectomy-first” strategy is proposed for osseous venous compression, pending prospective validation. View this paper
