Cover Story: Space exploration exposes astronauts to a unique radiation environment dominated by galactic cosmic rays and solar particle events, raising concerns about long-term cancer risk during extended missions beyond low-Earth orbit. This review examines current evidence on space radiation carcinogenesis, available risk-prediction models, and the major uncertainties related to dose estimation, radiation quality, and individual susceptibility. Particular attention is given to emerging imaging approaches and their potential role in monitoring radiation-induced biological effects and supporting personalized risk assessment. A better understanding of these mechanisms will be essential for protecting astronauts’ health during future missions to the Moon, Mars, and beyond. View this paper
