2026 August - 86 articles

Cover Story: Therapeutic failure in invasive fungal infections is rarely explained by antifungal resistance alone. This narrative review explores how host factors, pathogen characteristics, diagnostic limitations, antifungal exposure, and source control can contribute to a suboptimal response. It offers a structured approach to distinguish true from apparent failure, identify modifiable factors, and guide reassessment before treatment is escalated. Clinical algorithms, practical checklists, and potential quality indicators are also presented to support more informed decision-making and to examine treatment failure within the broader context of antifungal stewardship. View this paper
