- Makroangiopathie [20–40%) der hirnversorgenden Gefässe, sowohl arteriosklerotisch als auch nich-tarteriosklerotisch (z.B. Dissektionen, fibromuskuläre Dysplasien, Vaskulitiden u.a.) mit arterio-arteriellen Embolien oder hämodynamisch bedingten Infarkten.

- Mikroangiopathisch (20–40%) bedingte Gefässprozesse auf dem Boden arteriosklerotischer oder nicht arteriosklerotischer Veränderungen.

- Proximale Emboliequellen (25–40%), insbesondere die kardiale Embolie bei Vorhofflimmern oder kardialem Thrombus, seltener auch die paradoxe Embolie bei offenem Foramen ovale (PFO).
- 4
- Seltenere Ursachen umfassen insbesondere Gerinnungsstörungen, Vaskulitiden und hämatologische Erkrankungen.
