Ultrasound has been used as a diagnostic tool by neurologists since the 1980s, when the first pioneers observed changed patterns of muscle echotexture in patients with muscle dystrophy. Since then, vast enhancements in ultrasound resolution and quality have added to its utility as an inexpensive, painless, real-time diagnostic instrument. By now, the value of ultrasound in peripheral entrapment neuropathies has been firmly established. Improved understanding in the field of neuromuscular diseases e.g., inflammatory neuropathies and motor neurone diseases, has positioned ultrasonography as a parameter of nerve and muscle morphology to complement traditional electrodiagnostic studies. But there is a catch: training and experience are essential. This is why recognised experts in the field and beginners met at the first Nerve and Muscle Ultrasound Hands-on Teaching Course on 6th and 7th of March 2015 in Lucerne, convened by local senior consultant neurologist Dr. Anne-Kathrin Peyer-Kauffmann [1]. Transient global amnesia is an acute, benign, isolated and temporarily limited disturbance of memory, that can occur repeatedly but shows no increased risk of cardiovascular events or stroke in particular. Therefore, patients with the typical clinical presentation and a normal brain magnetic resonance-scan require neither further diagnostic nor therapeutic interventions. Since the differential diagnosis of transient global amnesia is wide, and transient ischaemic attacks can present similarly, a careful clinical evaluation and neuroimaging is recommended. In any case of doubt further diagnostic steps according to stroke workup should be initiated. In contrast, a transient ischaemic attack represents a neurological emergency where clinical and diagnostic evaluation must be introduced fast. The rapid establishment of therapeutic and secondary preventive measures decreases the clearly elevated stroke risk and prevents disability. Full article

Could extending sport-studies to all succeed as a preventative mental health strategy? Public mental health interventions must target a specifically vulnerable population, defined by a given context and time-frame. Schools have often been the elected medium for promoting physical health and more recently also mental health. This article presents an example intervention that targets students approaching the end of compulsory schooling. We were thus targeting students of a specific age considered vulnerable, affected by an external factor over which they have no control: puberty. In addition, given that the target environment consists of compulsory schooling, we were also able to involve a significant majority of all adolescents in this age group. On analysis of the scientific literature, it appears that participating in extracurricular activities, particularly those involving sport, constitutes one of the primary factors protecting against students dropping out. Team sports in particular stood out as being positively associated with decreased risk-taking in general, as well as reduced mental and physical health problems. These activities enable adolescents to act out particular issues relating to their daily lives, thus allowing them to progress in building their own identities. In addition to acting out such psychological issues, these activities also let them build up their social network, be it through interaction with their peers or even with adults. The Geneva “Sport-Art-Studies” programme consists of a collaboration between various orientation courses and sports clubs, enabling these young adults to keep up an intense training schedule while still following a near-normal curriculum. This model demonstrates the advantage in integrating sports practice into an adolescent’s schedule in a structured and coordinated way, while facilitating family functioning. In the interests of public mental health, this sports-studies model could also be adapted from a model for supporting young elite athletes to one aimed at promoting school integration with a preventative mental health objective. Despite year-long efforts to de-stigmatise the psychiatric field, the exclusion, rejection, and social distancing of patients with mental illness remains a critical issue in modern society. In addition to the most severely affected, namely people with mental disabilities, those working in the psychiatric profession, along with psychiatric institutions and psychiatric therapy, are still the object of stigma and prejudice in public life. This stigmatization may lead to resignation, the deterioration of psychiatric symptoms, and enhanced passivity. By empowering the psychiatric field and those affected with psychiatric disability, we may be able to reduce stigmatisation and, in collaboration with society and the political sphere, improve the real-life conditions of these patients. Empowerment may reactivate their feelings of usefulness and increase self-efficiency, giving them new hope and confidence in their own skills, along with a more optimistic view of life. We should strive to increase awareness of and access to the leading model of the well-informed and emancipated patient, as well as the efficacy of pharmacological and non-pharmacological treatment modalities, so that such outdated prejudices can be effectively overcome. By means of an integrated psychiatry providing more humanised patient-centred care, we may be able to reduce the stigma affecting psychiatric institutions. Nevertheless, psychiatric professionals and politicians must work in close collaboration to achieve effective and long-lasting improvements as regards the stigmatisation of patients with mental disorders in the communities where they live.
