Cover Story: This cover image shows a newborn infant with Tetralogy of Fallot, a common form of cyanotic congenital heart disease. Since the beginning of congenital cardiac surgery, the focus of clinical care has shifted from improving survival to preserving neurodevelopment and quality of life by as much as possible. Infants with congenital heart disease may develop neurologic sequelae, possibly affecting their later development. In this population, neurologic injury often occurs partly due to cerebral hypoxia or ischemia as a result of an underlying birth defect, in addition to the immaturity of cerebral vascularization. Monitoring cerebrovascular autoregulation could be a key to understanding and subsequently improving the neurological outcomes of this patient population.
