Cover Story: Acute idiopathic blind spot enlargement syndrome (AIBSES) predominantly affects young adults and is often misdiagnosed as optic neuritis because of low awareness. Only around 100 cases have been published. A careful taking of history and unprejudiced ophthalmological workup help in diagnosing AIBSES in patients with unilateral visual field loss in the blind spot area, acute onset photopsia, and funduscopically few or no optic disc changes. As it is a disease of the outer retina, optical coherence tomography (OCT) has become the gold standard in diagnosing AIBSES. In our case series, we present three consecutive patients with AIBSES that are followed prospectively with and without steroid therapy. All of the patients reported full recovery of their symptoms and partial restoration of the outer retinal layer anatomy. View this paper
