Dear Colleagues,

The role of healthcare is now shifting beyond disease treatment to focus on disease prevention, and it is even advancing into technologies that can reverse the aging process. Consequently, anti-aging and skin rejuvenation have become emphasized areas within the fields of dermatology, plastic surgery, and cosmetics. In this Special Issue, titled "Treatment for Anti-aging and Rejuvenation", we warmly invite your submissions of research and review papers encompassing topics such as cosmeceuticals, energy-based devices, laser modalities, surgical interventions, and more. Furthermore, we look forward to the sharing of up-to-date knowledge in the fields of cosmetology, dermatology, aesthetic dermatology, and plastic surgery.

Facial hyperpigmentation is a highly prevalent dermatological condition, characterized by dark spots on the skin resulting from excess melanin production. Hyperpigmentation significantly impacts patients’ quality of life and self-esteem. Current treatments often present disadvantages linked to poor product tolerability. A topical cosmetic approach combining three lightening active ingredients (tranexamic acid, niacinamide, vitamin C) offers a new option for treating dark spots on the skin. The present in-use test under dermatological control evaluated the clinical safety and efficacy of a cream and serum containing these three ingredients, formulated with hyaluronic acid for enhanced delivery, stability, and efficacy. A total of 22 Caucasian patients with facial hyperpigmentation, both male and female, aged between 45 and 67 years, applied the cream and serum for 8 weeks. Clinical assessments, colorimetric evaluations, standardized photography, and self-assessment questionnaires were used to measure outcomes. No serious adverse effects were recorded, and the incidence of local adverse effects was low, highlighting good tolerability of the investigated test items. In most participants, significant improvements in hyperpigmented areas were recorded. Clinical scoring by the dermatologist investigator indicated a statistically significant 13% reduction in color intensity and a 6% reduction in the size of dark spots after 8 weeks of treatment. Colorimetric evaluation showed a statistically significant 1% increase in luminosity (L* parameter) and an 8% improvement in the Individual Typological Angle (ITA°) in endpoint, indicating lighter skin spots. Subjective assessments reflected high user satisfaction, with 95% of participants noting improvements in skin hydration and luminosity, and 77% reporting a reduced appearance of dark spots. Overall, the present work supports the use of tranexamic acid, niacinamide, and vitamin C as an effective and well-tolerated combined topical management option for hyperpigmentation. This combination offers a viable alternative to classical whiteners for individuals seeking to reduce facial skin coloration imbalance and improve skin tone. Full article

The use of low-fluence picosecond (LFPS) 1064 nm neodymium-doped yttrium aluminum garnet (Nd:YAG) lasers, referred to as laser toning, is increasingly acknowledged as an effective treatment for pigmentation disorders in the Asian skin phenotype. This study aimed to conduct a comparative analysis on the effectiveness and safety of utilizing LFPS 1064 nm Nd:YAG lasers against picosecond 532 nm Nd:YAG lasers in treating pigmented lesions among Chinese patients. A retrospective photographic analysis and chart reviews were performed on 31 subjects exhibiting Fitzpatrick skin types III–VI who underwent LFPS 1064 nm Nd:YAG or picosecond 532 nm Nd:YAG treatments at a single tertiary center. Utilizing VISIA Complexion Analysis, comparative photographs were taken. Two independent physicians evaluated treatment efficacy using a visual analog scale (VAS) to assess the percentage of pigmentary clearance in standard photographs. Solar lentigines were the most prevalent pigmentary disorder, followed by post-inflammatory hyperpigmentation (PIH), nevus zygomaticus, melasma, freckles, and nevus of Ota. The clinical effectiveness of picosecond 532 nm and LFPS 1064 nm laser treatments proved comparable for lesions on the face, with mean VAS scores of 2.2 ± 1.1 and 1.8 ± 0.8, respectively. There were two cases of PIH in the picosecond 532 nm group, which resolved within one month. Overall, the LFPS 1064 nm laser demonstrates promise as a safe and efficient therapeutic modality for managing pigmented lesions in Chinese patients. Full article

Background: Laser-assisted drug delivery is a promising strategy that enhances topical treatment by increasing cutaneous permeation and reducing side effects. In cosmetic settings, the efficacy and safety profiles of a treatment must meet the need of a painless procedure with fast recovery. In this context, Q-switched laser appears promising as it can open cutaneous pores without creating a carbonisation barrier. Methods: A split-face study on patients presenting for cosmetic procedures at IDI-IRCCS, Rome (30 September–18 October 2023), was conducted. Pan-facial Q-Switched laser was followed by a topical biostimulator applied on half of the face. Post-procedure local reactions were recorded together with patients’ perceptions. Cutaneous elasticity and hydration were assessed at baseline and three-week follow-up. The Skindex17 questionnaire evaluated the procedure’s impact on patients’ life quality. Also, participants and physicians expressed satisfaction with the treatment. Results: The procedure was well tolerated by patients; local reactions include transitory erythema, superficial bleeding, and oedema, none of which had an impact on daily life. An improvement in skin quality was documented objectively and subjectively by patients and physicians. Conclusion: Q-switched lasers emerge as promising devices for drug delivery, especially for cosmetic reasons. Indeed, the low risk of local reactions together with a remarkable increase in cutaneous permeation make this a suitable strategy for cosmetic procedures. Full article

One of the most notable signs of an aging face is the nasolabial folds (NLFs), which often diminish emotional well-being and self-confidence. To address this concern, many people seek solutions to improve their appearance, often turning to fillers. The ULTRACOL100 device, a tissue restoration material, has been previously investigated and shown to exhibit significant efficacy in both in vitro and in vivo studies. In this research, we aim to explore the safety and effectiveness of the clinical trial of ULTRACOL100 in improving the skin in the NLF area over an 8-week observation period. Male and Female adults with nasolabial folds received two injections of ULTRACOL100, with a 4-week interval between treatments, on one side of their faces. On the other side, they received control materials (REJURAN®, JUVELOOK®, or HYRONT®). The assessment of skin improvement in the nasolabial fold area for each subject took place before and four weeks after each application. Various skin parameters, such as roughness, elasticity, moisture, transparency, trans-epidermal water loss, tone, radiance, skin pore size, and skin density, were measured to evaluate the outcomes. The application of the ULTRACOL100 device significantly reduced the skin roughness, the trans-epidermal water loss, and the skin pore size and increased the skin’s elasticity and internal elasticity, as well as the skin’s moisture, transparency, skin tone, radiance, and density. This study comprehensively investigates the effectiveness and safety of the ULTRACOL100 device, comparing it with three commercial products (REJURAN®, JUVELOOK®, and HYRONT®). The ULTRACOL100 device showed comparable performance in improving the appearance of the NLF area among this study subjects. Full article

This original article presents the findings of a comprehensive case series, shedding light on the efficacy of diverse treatment modalities for managing precancerous and cancerous skin lesions and their remarkable rejuvenation effects on the skin. A particular focus is placed on the promising outcomes achieved through the application of a combination treatment involving 5-Fluorouracil (5-FU) and salicylic acid, which demonstrates enduring and noteworthy results. Furthermore, alternative therapeutic approaches, including 5-FU monotherapy, Methyl aminolevulinate–photodynamic therapy (MAL-PDT), and the combination of Imiquimod therapy with MAL-PDT, exhibit substantial potential for patients seeking non-surgical solutions. These treatments manifest as valuable tools in improving skin texture and mitigating the effects of photodamage. Nevertheless, the intricate interplay between the chosen treatment, the extent of photodamage, and individual patient characteristics, with a particular emphasis on age, necessitates long-term follow-up to gauge treatment outcomes and the likelihood of lesion recurrence. Notably, these treatments are associated with a significant degree of inflammation, igniting curiosity regarding enhanced skin cellular turnover and the potential for a more youthful skin appearance. Our findings accentuate the promise of topical fluorouracil (5-FU) and photodynamic therapy (PDT) in combating photoaging among patients with actinic keratoses. However, a need for further in-depth research is evident to unravel the nuanced relationships between these treatments, the severity of photodamage, and the influence of patient-specific factors. Such comprehensive investigations are instrumental in optimizing patient care and outcomes, offering a holistic approach to managing photodamage within the context of actinic keratoses. This work, when combined with existing literature, provides valuable insights and serves as a catalyst for future research to fully unlock the potential of these treatments, ultimately enhancing the quality of patient care. Melasma, also known as chloasma and the “mask of pregnancy”, is a common acquired pigmentary disorder characterized by irregular, hyperpigmented brown-to-grey patches primarily situated on the face. It typically affects women of reproductive age, especially those exhibiting Fitzpatrick skin types III to V. The precise etiopathogenesis of melasma is complex and has not been fully elucidated; however, ultraviolet radiation, hormonal factors, and genetic predispositions significantly contribute to the melanin production increase associated with this condition. Due to the multifactorial aetiology, resistance to various therapeutic options, and high recurrence rate, treating melasma is challenging. Hydroquinone has long been considered a gold standard in melasma treatment due to its ability to inhibit tyrosinase; however, it has faced scrutiny after concerns about its adverse effects. Current treatment strategies include various topical and systemic therapies, procedural interventions, as well as combinations of these methods. For optimal results, both photoprotection and a treatment plan that targets different pathogenic mechanisms should be used. Additionally, treatment should be tailored to patient characteristics, such as skin type, the severity of the condition, and compliance. This review summarises current treatment options, focusing on long-term therapy and the latest advancements in managing this challenging condition. Full article

Dermal injections of hyaluronic acid gel for aesthetic skin rejuvenation are becoming increasingly popular nowadays. Although these products are classified as medical devices, the regulations on their administration by licensed practitioners are still weak, whereas their manufacturers increasingly highlight and advertise the cellular effects that underpin the efficacy of these injections. In this review, we discuss all current knowledge on the mode of action of dermally injected hyaluronic acid and the potential toxicological implications, especially from crosslinked gels, in conjunction with the current global regulations. We also highlight the urgent need for further research to elucidate the therapeutic implications and underscore the imperative need for robust regulatory frameworks to safeguard public health. We conclude that dermal injections of hyaluronic acid have several therapeutic implications that warrant further research and that strict regulations must be applied to their manufacture/quality control and the required qualifications of licensed aesthetic injectors. Vitamin D is a natural photoproduct that has many beneficial effects on different organs, including skin. Active forms of vitamin D and its derivatives exert biological effects on skin cells, thus maintaining skin homeostasis. In keratinocytes, they inhibit proliferation and stimulate differentiation, have anti-inflammatory properties, act as antioxidants, inhibit DNA damage and stimulate DNA repair after ultraviolet (UV) exposure. In melanocytes, they also inhibit cell proliferation, inhibit apoptosis and act as antioxidants. In fibroblasts, they inhibit cell proliferation, affect fibrotic processes and collagen production, and promote wound healing and regeneration. On the other hand, skin cells have the ability to activate vitamin D directly. These activities, along with the projected topical application of vitamin D derivatives, are promising for skin care and photo protection and can be used in the prevention or possible reversal of skin aging. Background: In recent years, social media and AI have indirectly taken control of our daily lives. We bring attention to the impact that social networks, photo-editing applications, and artificial intelligence have on potential patients when they are looking for a surgeon for a possible cosmetic surgery, as well as the criteria they consider in relation to the interest in the use of the internet by surgeons. Methods: A systematic review of the past 10 years (2014–2024) was conducted following the PRISMA structure. PubMed and Google Scholar were searched for articles containing the following terms: plastic/esthetic surgery, social media, AI, filters, dysmorphia. All articles were saved using Zotero software version 6.0.37. We reported a prospective study including a 141 patients applying for esthetic surgical interventions in the time interval between February and October 2021. It also involved 44 esthetic surgeons from Tunisian clinics. The influence of social media was evaluated using questionnaires made based on the literature. Results: Using the keyword search, 71 articles were found. A total of 19 articles were selected for data extraction. It was observed that in the last 3 years, the literature has focused on photo-editing and AI in the cosmetic surgery field. A total of 107 patients chose their surgeon based on a surgeon’s social medias rather than their reputation and their website. Conclusions: The increased advancements of the internet have clearly influenced decision making in the field of cosmetic surgery. Full article

The pursuit of youth and attractiveness is a fundamental desire in contemporary society. For many individuals, the appearance of visible signs of aging, such as skin laxity, gravitational changes in facial and neck soft tissues, and loss of harmonious proportions, can be psychologically distressing. The choice of effective methods for addressing these changes depends on their specific characteristics, the patient’s anatomical features, and the type of facial aging. The introduction of 3D imaging to determine the hyoid bone level, the volume of soft tissue structures in the cervico-mental area, ptosis of subplatysmal structures, and the volume of submandibular glands, in addition to ultrasound study of the submental projection at the preoperative stage, are key for making the best decisions regarding preoperative planning. This case report aims to illustrate how modern imaging techniques are a fundamental part of the preoperative assessment of the mid and lower third of the face to ensure the most favorable aesthetic outcomes for each patient.
