Cover Story: Clinicians attempt to avoid antibiotic use unless there are convincing signs of infection. The aims of this study were to report the ontogeny of biomarkers for inflammation among 142 infants born at 22 to 32 weeks of gestation and contextualize the levels with common etiologies of preterm delivery. Independent of gestational age, monocyte chemoattractant protein-1 (MCP-1), interleukin-6 (IL-6) and C-reactive protein (CRP) sharply declined following delivery. MCP-1 was increased in infants exposed to preeclampsia but decreased in those exposed to chorioamnionitis. CRP was increased in infants with funisitis, but it increased a day later than IL-6 in those with sepsis. Our results reveal unique biomarker profiles in infants exposed to distinct perinatal complications with implications for antibiotic stewardship and mechanistic investigations. View this paper
