Cover Story: Uncertainty is inherent to rare diseases, especially multifactorial ones, mainly due to complex gene–environment interactions. Case studies (SARS-CoV-2-associated acute multisystem inflammatory syndrome in adolescents; neural tube defects; familial Mediterranean fever) highlight the added value of a systematic uncertainty analysis, pinpointing critical areas for research and healthcare. The rare disease community can use its strengths (stakeholder involvement, data sharing, open science) to successfully face this great challenge. View this paper
