Cover Story: Pseudoarthrosis, the failure of normal fracture healing, affects 10–15% of long bone fractures, causing pain, disability, and the need for repeated surgeries. Despite advances in understanding bone healing, diagnostics still rely on clinical and radiographic assessments, lacking precise predictive tools. This review explores the genetic and biological basis of pseudoarthrosis, highlighting key mechanisms such as mesenchymal stromal cell dysfunction, altered BMP signaling, and genetic polymorphisms. It emphasizes the need for future research to develop innovative diagnostics and personalized therapies. View this paper
